BEST Trial: Bone and Joint infection – Simplifying treatment in Children Trial
The BEST trial (Bone and Joint infection – Simplifying treatment in Children)
The BEST trial is a multicentre randomised control trial in Australia and New Zealand that aims to determine whether treatment of bone and joint infections in children with oral antibiotics alone is just as effective as treatment with intravenous antibiotics followed by an oral antibiotic course.

This clinical trial is to compare the use of oral fosfomycin against standard of care antibiotics for the treatment of antibiotic resistant urinary tract infections in children.
The main questions the trial aims to answer are:
- Is oral fosfomycin non-inferior in efficacy to the current standard of care for the treatment of antibiotic-resistant urinary tract infections in children?
- Is oral fosfomycin a safe and well-tolerated antibiotic in children?
- What is the best dosing regimen of oral fosfomycin for the treatment of antibiotic-resistant UTIs in children?
O-SNAP: Optimizing antibiotic dosing in the Staphylococcus aureus Network Adaptive Platform trial
This sub-study aims to find out how much antibiotic we should be giving to adults and children with Staphylococcus aureus blood stream infection (SAB), so that they get the best treatment result.

This is a multicentre, pragmatic, adaptive, two-arm, open label randomised control trial at seven major paediatric hospitals in Australia and New Zealand aims to determine in immunocompromised children with cytomegalovirus viraemia whether individualised intravenous ganciclovir dosing using a web app is superior to standard intravenous ganciclovir dosing in achieving virological clearance.
Advancing the evidence base for off-label medications in children
A multicentre prospective audit with an embedded pharmacokinetic and pharmacodynamic study to determine the dosing regimens, safety and efficacy of off-label medications used in children.
Audit of therapeutic drug monitoring of Vancomycin in paediatric tertiary hospitals
A multicentre retrospective and prospective audit aiming to develop a population pharmacokinetic-pharmacodynamic model of vancomycin in children ≤ 18 years and use this to create personalised vancomycin dosing and AUC24 calculators.
A pharmacokinetic study of fluoroquinolones using capillary microsampling
This study aims to determine the pharmacokinetics of fluoroquinolones in children and to validate capillary microsamples for determination of fluoroquinolone concentrations.
Future directions – scoping literature review for antimicrobials for drug-resistant bacterial infections in children
A review of the epidemiology, current treatment recommendations and overview of paediatric data of drug-resistant bacterial infections in children.
